Objectives

Complete a grade sheet that survives audit; state the audit chain and the instructor’s role in it; state the records test.

The records test

“For any pilot, on any date, produce the evidence they were qualified for that flight.” Every document you sign either supports that sentence or wastes paper.

What a defensible grade sheet contains

• Tasks by PQS row ID — never freehand task names.
• A grade per task from the published scale, plus intervention notes.
• The PQS revision in effect, the date, aircraft, location, and both signatures.

The audit chain

Grade sheet → PQS revision → source manual revision. When Engineering changes the aircraft and Tech Pubs revises the manual, the standard revises, and every grade sheet after that date points at the new revision. An auditor — or an accident board — can walk the chain in either direction. Instructors are the first link: an unsigned, undated, or freehand grade sheet breaks the chain at its origin.

Timeliness

Grade sheets are completed the day of the event. A grade sheet reconstructed from memory a week later is a work of fiction with a signature on it.
